Texas Health Form Requirement

Fill out Admission Health Requirements as required by the Texas Dept. of Family and Protective Services with either: 1) HEALTH CARE PROFESSIONAL STATEMENT (health care provider signs form) Or 2) A signed and dated copy of your health care professional’s statement that says your child is physically able to participate in the program. A signed statement from your health care professional replaces the signature on the form.

I understand my CEC registration will not be fully considered until the TX DFPS Form 2935 and a $100 deposit (cash or check) is received by the Trinity School office.
